Forms in MS Excel Format
(RECOMMENDED):
Form 1:
For recording general information and observations of
hawks (filled out for each visit).
Form 2:
For Swainson's Hawk observations (filled out if you see Swainson's
Hawks).
Form 3:
(Optional) For recording shrubland and grassland birds of interest.

(ALTERNATIVE):
Form 1:
For recording general information and

The Excel Forms can be
filled out electronically on your computer or printed out for use in
the field.
Most people prefer to fill
out the forms in the field, then enter them again on the computer
and e-mail the data.
Typing in your data will
make it easier for others to read and avoid the need to mail in
forms or make photocopies.
The PDF Forms are slightly
less easy to read and should only be used if you cannot open the
forms in MS Excel. The PDF forms can only be printed and
filled out by hand. |
Submitting Forms and
Observations
Electronic versions of forms (preferred) can
be e-mailed to:

If you mail us your forms, we
recommend that you make copies in case the forms get lost in
transit.
